Instructions:
Fill in the information on lines 1-7 below. In the comments field enter the names of any guests who need gluten-free meals on the bus tour or at the Saturday evening banquet. If you are traveling to/from Burlington, IA by train, enter the information in the Amtrak Travelers section. Then press the 'Continue To Page 2' arrow button.

On page 2 you may add up to five guests. With each guest, select the events they wish to join, and their meal choices.

I would like to assist during the meet and/or shuttle Amtrak guests to the hotel.
Drivers and vehicles are needed Friday morning and evening and also on Saturday morning to shuttle guests arriving by Amtrak. See times below.
